What is MyChart and how does it work?
MyChart is just one of the ways UI Health is working to improve your healthcare experience. Schedule, reschedule, or cancel appointments – no phone call required. You can even check-in for your appointment before you arrive. MyChart has your after-visit summaries, test results, and discharge instructions, all in one place.
How do I set up my MyChart account?
Enter your activation code, your email address on file at UI Health, and Date of Birth. MyChart username. This should be something that others wouldn’t be likely to guess but easy for you to remember. It cannot be changed at any time. Your password must be at least eight characters and must be different from your MyChart username.
What can I do with my UChicago Medicine MyChart account?
With your UChicago Medicine MyChart account, you can: Schedule, reschedule or cancel your next appointment, and view other details of upcoming appointments directly from your phone or computer. For your convenience and safety, we offer secure and easy virtual visits for most non-urgent visit types.

The 21st Century Cures Act is a federal mandate that is designed to give patients more access to their medical information. As of March 31, 2021, UI Health is providing test results and provider notes (with a few exceptions) will be released via the MyChart patient portal as soon as they are available.
