In just a few simple clicks, there is so much you can do and access with your MyChart account. Sign up online to get started and download the MyChart app for your phone.

How do I sign up for MyChart?

Go to MyChart and select “Sign Up Now.” You will need your access code and medical record number. This can be found on the last page of the after-visit summary you received at the end of your appointment. If you lost your access code or do not have one, please contact the MyChart help desk at 503-494-5252 .

When will I see my results in MyChart?

We release ALL results to you through MyChart as soon as they are available. You may see some results even before we do. Please give us 2 business days to review and let you know our thoughts. We look at every result. We will contact you with any results that concern us.
