How do I request a personal invitation to CMC patient portal?
If you manage the health of a patient, talk to that patient’s health care provider during the next visit to request a personal invitation to CMC Patient Portal. Self-enrollment is not currently available to caregivers or guardians, but portal access is available if requested at the hospital or office. * Indicates a required field.
