CommUnityCare Health Centers is proud to introduce a new medical record system that will bring many new benefits to you, the patient. Part of this system is an application called MyChart. This is an easy-to-use secure application that gives you access to your health information.

How do I get access to my MyChart account?

You will need to enter your full name, address, birthdate and legal sex. You must enter the activation code on the MyChart website along with your zipcode and birthdate to gain access to your MyChart account. “I, PATIENT NAME received and consent to the CUHCC consents in this email.”
