How do I access MyChart?
MyChart is accessible on your desktop or mobile web browser at mychart.dulyhealthandcare.com or download the free Duly Health and Care app on your smartphone or tablet. You can also securely access the MyChart account for family members or persons under your care with Proxy access. Complete the Proxy Access forms available here to receive access.
What are MyChart alerts?
When you first login to MyChart, you provide an e‑mail address for alerts. These alerts will not contain specific information from your MyChart account; rather they are notifications that you have updated information to review in MyChart (such as new test results, a message or appointment reminder).
Can a child under 12 access MyChart?
Yes, this is called “ proxy” access. For a child under 12, this allows a parent or legal guardian to log into their personal MyChart account, and view information regarding their child.
