How do I access my Patient Portal?
Visit the Medical Records Department at East Alabama Medical Center or EAMC-Lanier with your photo ID and a completed patient portal authorization form. A valid email address is required for accessing your patient portal. If you can’t make it to the Medical Records Department, you can mail or fax the form with a copy of your picture ID.
