How do I submit a patient portal agreement?
When completed, you may either mail the form to Family Practice Group or bring it into the clinic at your earliest convenience. Mailing address: Family Practice Group, 229 Stewart Avenue, Medford, Oregon 97501 Patient Portal Agreement for an INDIVIDUAL Portal Account (for an individual who does not share an email address).

It has been known that spam blocking software used by email providers such as AOL and Netzero may be blocking legitimate emails.
How do I get a patient portal code?
You may also receive the code over the phone once we have received your Patient Portal Agreement via mail by calling 541-779-5531. The code will be emailed to you via our secure system. This code allows you to log on and enroll to create your new online patient account, user ID and password.
