How do I sign up for myuhealthchart?
MyUHealthChart is a free service offered to our patients. To sign up, you can request an activation code at your next clinic visit, or, you can call us at 877-448-1773. Once you have the activation code, to sign up: Click on the Sign Up Now link in the Sign In box. You will see the New Member Sign Up page. Enter your MyUHealthChart Access Code.
