What is MyChart and how does it work?

MyChart is a secure online portal that allows you to access your electronic medical record and send messages to your doctor directly without going to the clinic or picking up the phone. You can also: No more waiting for a phone call or letter — view your results and your doctor’s comments within days.

How do I Reset my MyChart username or password?

If you have had a MyChart account as a patient at: OHSU, Mid-Columbia Medical Center, Tuality Healthcare, or Adventist Health Portland please select recover usernameor reset password. If you have never had a MyChart account at any of these hospitals or clinics, please click Sign Up Online.

What if I don’t have a MyChart account?

If you do not have a MyChart account of your own, first, you will need to create your own account even if you are not a Hillsboro Medical Center patient. Then, you can log in and set up a proxy account for your child or family member.

To get parent/caregiver proxy access to another person’s MyChart account, you can request this access upon checking in at the clinic or call the clinic directly. For children under 15 years of age, only parents or legal guardians may request access to their child’s account.
