Get the MyChart app for access to your lab results, appointment information, current medications, immunization history, and more on your mobile device. To install the app, visit the App Store or Google Play to download. Search for “MyChart” and choose the MyChart Epic App.
What is MyChart activation code?
Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to login with your own username and password. If you were not issued an activation code, you can call your clinic or request an access code here.

What is a MyChart app?
MyChart is a free app and web portal for all Yakima Valley Farm Workers Clinic patients to access their health information. Simply download the MyChart app (for iOS or Android ), or log in online.
How secure is MyChart?
Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. To protect yourself, do not give out your username and password. MyChart will never send your actual health information via email; however, you will receive communication alerts via email to inform you when new results and other communications are sent to your MyChart.
What to do if you don’t have refills?
