This program allows Gundersen to extend their electronic health records to Crossing Rivers Health and patients in each organization’s care. Have an existing Gundersen Health MyChart account? How do I sign up for Gundersen Health?

Review your health records, including notes, medications, allergies and immunizations. Do all the above for family members, dependents (age 18 or older) or friends who give their permission. Step 1: Go to mychart.gundersenhealth.org and click Sign Up Now Step 2: Enter the activation code, if you have one.
